Treatment of Stage III Favorable Histology Wilms Tumor in Children — Standard Risk
Clinical Scenario
This protocol is for children with stage III favorable histology Wilms tumor (FHWT) who are classified as standard risk following initial risk assessment. Stage III disease reflects local tumor extension beyond the kidney, and favorable histology is a defined pathologic category that carries specific treatment implications in this age group.
Patient Population
Eligible patients are children with a confirmed kidney mass, confirmed as Wilms tumor with favorable histology at local stage III. Standard-risk classification is determined after completing the initial risk assessment and drives the selection of the treatment pathway described in this protocol.

- DD4A is recommended for patients with stage III FHWT classified as standard risk after the initial risk assessment.
- Flank RT or WAI is recommended for patients with local stage III.
- If RT is being considered, the timing of RT should be coordinated with chemotherapy to avoid the coadministration of full doses of dactinomycin or doxorubicin with RT.
