Treatment of Whipple’s Disease with Sulfonamide Allergy and Tropheryma whipplei Endocarditis
Clinical Scenario
This protocol covers Whipple’s disease presenting as Tropheryma whipplei endocarditis in a patient with a sulfonamide (sulfa) allergy. The allergy rules out standard sulfonamide-containing regimens and requires a specifically adapted treatment strategy.
Treatment Approach
An antibiotic combination tailored for sulfa-allergic patients is indicated. In select cases, surgical management of the affected valve may also be required.
The complete regimen — agents, sequencing, and full clinical decision points — is available in the structured protocol below.
References
Patients with a sulfonamide allergy should be treated with a combination of doxycycline and hydroxychloroquine.
Valve replacement surgery may be needed to treat T. whipplei endocarditis.
