What is the Treatment of Classic Whipple's Disease?

This protocol addresses classic Whipple disease — the typical systemic presentation
in which patients require a structured, staged antibiotic regimen to achieve disease control.
Treatment Approach
Management follows a two-phase antibiotic strategy: a short course of intravenous therapy
to establish rapid disease control, followed by a prolonged period of oral maintenance treatment.

Treatment Goals
- Resolution of arthralgias and fever within a few days of initiating therapy
- Resolution of diarrhea and malabsorption within 2–4 weeks

- Classic Whipple disease.
- The standard initial treatment regimen is either (1) intravenous (IV) ceftriaxone 2 g once daily OR (2) 2 million units of penicillin G IV every 4 hours, both for 14 days' duration.
- After the initial treatment is completed, 1 year of maintenance therapy is with oral double-strength trimethoprim-sulfamethoxazole twice daily.
- Arthralgias and fever usually resolve within a few days.
- Diarrhea and malabsorption disappear within 2–4 weeks.
