Warm autoimmune hemolytic anemia: what to do when corticosteroid therapy has not stabilized hemoglobin

Clinical scenario

This protocol applies to patients with warm autoimmune hemolytic anemia who have undergone initial corticosteroid treatment but have not achieved — or cannot maintain — the required hemoglobin target, with ongoing evidence of hemolysis.

Prior treatment and failure condition

The preceding regimen was built around prednisone. The intended outcome was a stable hemoglobin above 10 g/dL with a sustained rise in hemoglobin and resolution of symptoms. When those goals are not reached, or cannot be maintained at an acceptably low corticosteroid dose, escalation to this next-line protocol is indicated.

Next-step treatment approach

The next phase involves a targeted biologic agent — rituximab — added while the corticosteroid course is gradually wound down. The precise schedule, timing, and sequencing are detailed in the full protocol.

Treatment goals

Normalization of hemoglobin with no evidence of ongoing hemolysis — reflected by normal bilirubin, LDH, haptoglobin, and reticulocyte levels.
