Treatment of Squamous Cell Carcinoma of the Vulva, Stage T1, with Depth of Invasion ≤1 mm
This protocol defines the evidence-based management approach for adult patients with squamous cell carcinoma of the vulva classified as stage T1 and with a depth of invasion at or below 1 mm — a threshold that has direct implications for how broadly treatment must extend.
Clinical Scenario
Squamous cell carcinoma of the vulva, stage T1, depth of invasion ≤1 mm, in adults. The depth-of-invasion measurement is the pivotal criterion: it determines the extent of local surgical management and, critically, whether regional lymph node territory requires any intervention.
Treatment Approach & Goals
The recommended management is a surgical approach targeting the local vulvar tumor, with the clinical objective of achieving histologically confirmed clear margins at the resection site.

These guidelines apply to adults over the age of 18 years with squamous cell carcinoma of the vulva.
Tumors with depth of invasion ≤1 mm do not need groin treatment.
Radical local excision is recommended with the aim to obtain histological tumor-free margins [III, B].
