Vitreous haemorrhage in the setting of an underlying retinal detachment or retinal break represents a time-sensitive presentation. Identifying the structural cause drives the urgency and the choice of intervention.
This protocol applies when vitreous haemorrhage is associated with a concurrent retinal detachment or a retinal break. The presence of either finding significantly changes the management approach and its urgency.
When retinal detachment or a break is identified, urgent surgical intervention is indicated. The procedure also addresses the retinal pathology — the complete regimen, including how breaks are managed intraoperatively, is detailed in the full protocol.
Vitrectomy is performed urgently when a retinal detachment or break is identified.
Retinal breaks are sealed with cryotherapy or laser photocoagulation.
