First-Line Topical Treatment for Limited Vitiligo

Vitiligo with limited cutaneous involvement is a common clinical scenario requiring a structured topical approach tailored to lesion location and extent. First-line management aims for meaningful repigmentation while minimising local adverse effects.

Treatment Approach

First-line care centres on topical therapies, with agent selection depending on where the lesions are located — the face, neck and skin-fold areas are treated differently from extrafacial sites. A topical agent with regulatory approval specifically for vitiligo repigmentation is also part of the options.

Treatment Goal

The primary target is repigmentation of vitiligo lesions. Facial and neck lesions typically show the best response; the initial assessment of treatment progress is made at 3 months.

TCIs are first-line treatment in adults and children with limited involvement, especially for lesions in the face, neck and body folds with thin skin (e.g. inguinal, axillary regions).
A twice-daily application of TCI can be recommended (Radakovic-Fijan et al., 2009).
The treatment should be prescribed initially for 6 months.
When effective, prolonged treatment (e.g. up to 12 months or more) can be proposed.
In adults and children, once-daily application of potent TCS is recommended for patients with limited involvement.
Most studies used potent to very potent corticosteroids once daily, applied topically for 3–6 months.
The risk of local side effects (skin atrophy, telangiectasia, hypertrichosis, acneiform eruptions and striae) can be reduced by using an intermittent/alternating treatment scheme (e.g. 2 weeks on 2 weeks off) and will enable longer treatment periods.
The topical JAK-inhibitor ruxolitinib is now the first treatment approved for the repigmentation of vitiligo.
TCI monotherapy can induce ≥25% repigmentation in 55.0% of patients and ≥75% repigmentation in 18.1% after 3 months.
