Treatment of Ventral Hernia in Patients on Peritoneal Dialysis

When an umbilical or epigastric hernia is identified in a patient undergoing peritoneal dialysis, specific repair considerations apply. The dialysis context influences both the timing and the preferred surgical technique.

This protocol covers umbilical or epigastric hernia in patients on peritoneal dialysis. Umbilical hernia occurs in a notable proportion of this population — reported in 3–15 per cent of cases in retrospective series — making a defined management approach in this group clinically important.

Surgical repair is recommended. The preferred technique is an open repair with mesh placement. The complete protocol specifies the recommended mesh positioning method, approach to the peritoneal cavity, and guidance on operative timing in relation to active peritoneal dialysis.

The frequency of umbilical hernia in patients having peritoneal dialysis is reported to be 3–15 per cent in retrospective case series.

An open umbilical or epigastric hernia repair using onlay or preperitoneal mesh placement is suggested for patients on peritoneal dialysis.

It is suggested to repair an umbilical or epigastric hernia before initiation of peritoneal dialysis. It seems safe to perform the hernia repair during peritoneal dialysis.
