This protocol applies to patients presenting with a primary lumbar hernia — a defect in the lumbar region bounded by the 12th rib superiorly, the iliac crest inferiorly, the free border of the external oblique muscles anteriorly, and the vertebral column posteriorly.
Given the rarity of lumbar hernias, referral to a specialised hernia centre is an important early consideration. Surgical repair involves mesh-based reconstruction, with both open and minimally invasive approaches described.

A primary lumbar hernia is a defect in the lumbar region, which is bounded by the 12th rib above, the iliac crest below, the free border of the external oblique muscles anteriorly, and the vertebral column posteriorly.
As lumbar hernias are rare, it is suggested to consider referring the patient to a specialized hernia centre.
A lumbar hernia may be repaired by an open approach with preperitoneal mesh placement or by a laparoscopic approach with preperitoneal or intraperitoneal mesh placement.
