When conservative therapy does not relieve symptoms in patients with symptomatic chronic deep vein obstruction or stenosis of the iliocaval segment, a structured next-line protocol applies. The threshold for escalation is based on both anatomical severity and clinical impact.
Symptomatic chronic deep vein obstruction or stenosis of the iliocaval segment with at least 50% lumen area stenosis confirmed on intravascular ultrasound (IVUS). Intervention is considered when significant anatomical narrowing is accompanied by severe, lifestyle-limiting symptoms.
The previous line of management was conservative therapy: compression therapy, venotonic medications, lifestyle changes, and weight loss where applicable.
This protocol is indicated when that approach failed to achieve reduced leg discomfort or a meaningful reduction in edema volume.
The next step involves an endovascular intervention targeting the iliocaval segment, combined with continued conservative management. Imaging guidance is a central element of this approach.
Treatment goals include improved wound healing rate and measurable improvement in symptoms (VCSS score).
