Treatment of Varicose Veins of the Lower Extremities in Adults with Perforator Vein Reflux and Venous Leg Ulcers

This protocol covers management of lower-extremity varicose veins in adults where perforator vein reflux is present alongside active or recurrent venous ulceration — a presentation that requires both ulcer-focused wound care and attention to the underlying venous pathology.

Adults with documented perforator vein reflux in the lower extremities associated with venous leg ulcer(s). The presence of ulceration distinguishes this population and directly shapes both wound-care priorities and vascular treatment decisions.

The principal objective in this setting is healing of the venous leg ulcer. Achieving durable ulcer closure requires addressing the underlying venous insufficiency in conjunction with direct wound management.

First-line management centres on conservative measures including compression and comprehensive wound care, alongside venotonic medications and lifestyle modifications. The complete structured regimen — including additional interventional considerations specific to perforator reflux with ulceration — is detailed in the full protocol.
