Treatment of Varicocele When Sperm DNA Fragmentation Is Raised and Infertility Is Otherwise Unexplained

This protocol applies to men with a confirmed varicocele and elevated sperm DNA fragmentation whose infertility has no other identified cause — or who have experienced failure of assisted reproductive techniques, including recurrent pregnancy loss, failure of embryogenesis, or failure of implantation.

Clinical scenario
Varicocele with raised sperm DNA fragmentation in the context of otherwise unexplained infertility, or prior ART failure — encompassing recurrent pregnancy loss, failed embryogenesis, and implantation failure.
Treatment approach (partial)
Surgical correction of the varicocele is considered in this population, with a preferred microsurgical technique. The complete selection of approaches, procedural criteria, and the full management algorithm are available in the protocol below.

Clinical goal: Reduction in sperm DNA fragmentation.

Varicocelectomy may be considered in men with raised sperm DNA fragmentation with otherwise unexplained infertility or who have suffered from failure of assisted reproductive techniques, including recurrent pregnancy loss, failure of embryogenesis and implantation.

Varicocelectomy can improve sperm DNA integrity.

Current evidence indicates that microsurgical varicocelectomy is the most effective among the different varicocelectomy techniques.
