Varicocele in Adolescent Age with Persistent Ipsilateral Testicular Size Difference

This protocol applies to adolescent patients with varicocele in whom a persistent ipsilateral testicular size difference of greater than 2 mL or 20% has been confirmed on two separate clinical assessments performed six months apart, and/or who present with abnormal semen analysis findings.

Clinical Scenario

In adolescent age, a varicocele becomes an active management indication when the ipsilateral testis shows a documented, persistent size discrepancy — confirmed over two visits six months apart — or when semen analysis is abnormal. Prophylactic treatment is indicated when testicular growth deterioration is confirmed through serial clinical or Doppler ultrasound examinations and/or abnormal semen analysis.

Treatment Approach

Surgical correction of the varicocele is the indicated approach in this setting. Microsurgical techniques are considered the most effective among available options, and several procedural alternatives also exist. The specific selection criteria, preferred technique, and full management algorithm are contained in the complete protocol.
