Treatment of Localised Primary Uveal Melanoma Amenable to Eye-Preserving Treatment

In patients with localised primary uveal melanoma where anatomy and tumour characteristics permit globe-conserving approaches, the goal is to achieve disease control while preserving the eye. The specific clinical scenario — extent, location, and the feasibility of eye-preserving techniques — shapes the choice among available interventions.

Localised primary uveal melanoma in a setting amenable to eye-preserving treatment. When eye-preserving techniques are possible, radiation-based options should be preferred over surgical resection approaches.

Management of local recurrence in this setting may involve radiation-based or ablative interventions, selected according to the extent and location of recurrence. The full sequencing and selection criteria are detailed in the complete protocol.

If eye-preserving techniques are possible, RT options should be preferred over endo- and transscleral resections [I, B].

There is no current standard treatment of local recurrence. TTT, re-RT or enucleation can all be recommended as feasible and effective treatment options, depending on the extent and location of the recurrence [V, B].
