Treatment of Uterine Sarcoma in NTRK Fusion-Positive Gynecological Sarcoma

This protocol addresses uterine sarcoma arising in the context of an NTRK fusion-positive gynecological sarcoma — a rare subgroup of spindle cell tumors histologically resembling fibrosarcomas, defined by the presence of an underlying NTRK gene fusion.

The presence of an NTRK fusion places this tumor within a molecularly defined category. In locally advanced disease, the treatment approach is shaped by this molecular profile and the goal of enabling surgical resection with curative intent.

For locally advanced disease, the protocol considers systemic treatment or radiotherapy as a means to facilitate subsequent resection. Among available systemic options, NTRK inhibitors are associated with the highest reported response rates in this setting.
