Low-grade uterine adenosarcoma without sarcomatous overgrowth is a recognised histological sub-type of uterine sarcoma. When disease recurs or metastasises, hormone receptor status is a key determinant of the first-line treatment strategy.
This protocol applies to patients with low-grade uterine adenosarcoma without sarcomatous overgrowth. Management follows a similar framework to other low-grade uterine sarcomas, with treatment continued until disease progression or unacceptable tolerability.
For recurrent or metastatic disease that is estrogen/progesterone receptor positive, endocrine therapy is the established first-line approach — the specific options and sequencing are detailed in the full protocol.

Low-grade adenosarcoma should be managed similarly to patients with LG-ESS, with endocrine therapy until disease progression unless associated with unacceptable adverse effects.
Endocrine therapy with an aromatase inhibitor, progestogen, or GnRH analogs should be offered to patients with recurrent/metastatic estrogen/progesterone receptor positive low-grade uterine adenosarcoma (IV, C).
