This protocol covers patients diagnosed with low-grade uterine adenosarcoma without sarcomatous overgrowth — a distinct histological presentation of uterine sarcoma that carries specific management implications distinct from higher-grade or sarcomatous-overgrowth variants.

Specific Population

Low-grade uterine adenosarcoma without sarcomatous overgrowth. Hormone receptor status and extent of surgical resection are key determinants in defining the post-operative management strategy for this subtype.

Treatment Approach (Overview)

For completely resected, hormone receptor-positive disease at higher stages, post-operative endocrine-based therapy is a central consideration. Low-grade adenosarcoma should be managed similarly to patients with LG-ESS, with endocrine therapy until disease progression unless associated with unacceptable adverse effects.
Post-operative endocrine therapy can be considered in patients with stage II, III–IV completely resected estrogen/progesterone receptor positive uterine adenosarcoma (IV, C).
Post-operative radiotherapy could be considered in stage II–IV uterine adenosarcoma for local control, although there is no evidence to support a survival benefit (IV, C).
