Treatment of Uterine Sarcoma in High-Grade Endometrial Stromal Sarcoma (HG-ESS) or Undifferentiated Uterine Sarcoma (UUS)
High-grade endometrial stromal sarcoma (HG-ESS) and undifferentiated uterine sarcoma (UUS) are rare, aggressive subtypes of uterine sarcoma. Together they represent a small fraction of all uterine sarcomas, and their management in the relapsed or metastatic setting requires a specific systemic approach.
Clinical scenario
This protocol addresses patients with high-grade endometrial stromal sarcoma or undifferentiated uterine sarcoma in the relapse or metastatic setting, where first-line systemic therapy is being initiated.
Treatment approach (partial overview)
First-line systemic therapy involves chemotherapy-based regimens selected according to prior treatment history and patient fitness — including anthracycline-based options and combination regimens.
The full regimen, sequencing, eligibility criteria, and all options are detailed in the complete protocol.
References
- HG-ESS and UUS collectively represent about 5% of uterine sarcomas.
- First-line systemic therapy options include doxorubicin (if not used in adjuvant setting) combined with ifosfamide or as a single agent (III, B); or gemcitabine with docetaxel for a patient not able to receive doxorubicin (IV, C).
