Treatment of Uterine Sarcoma in High-Grade Endometrial Stromal Sarcoma or Undifferentiated Uterine Sarcoma

Clinical scenario

This protocol addresses patients with uterine sarcoma classified as high-grade endometrial stromal sarcoma (HG-ESS) or undifferentiated uterine sarcoma (UUS) — two aggressive subtypes that together account for a small but distinct minority of uterine sarcomas.

Treatment approach

For stage I disease, surgical intervention is central to management. The full structured regimen — including the role of additional modalities — is detailed in the complete protocol.

Specific sequencing, adjuvant considerations, and evidence-based recommendations are available via the full protocol below.

References

HG-ESS and UUS collectively represent about 5% of uterine sarcomas.

For stage I disease, total hysterectomy is the standard treatment.
