Treatment of Nonmetastatic Undifferentiated Pleomorphic Sarcoma

Undifferentiated pleomorphic sarcoma (UPS) presenting without distant metastasis represents a localised disease setting where a combined local treatment strategy is central to management. This protocol addresses the clinical approach in nonmetastatic disease.

Patients present with nonmetastatic undifferentiated pleomorphic sarcoma — disease confined without evidence of distant spread. The majority of these tumors are high grade, which shapes the treatment considerations.

Surgery performed by an experienced surgeon, combined with radiotherapy, forms the cornerstone of treatment in this setting. For high-grade tumors, perioperative chemotherapy is among the options considered — the full sequencing, selection criteria, and regimen details are in the complete protocol.

  • As for other STSs, surgery performed by an experienced surgeon plus radiotherapy remain the cornerstone of treatment of nonmetastatic tumors.
  • With the majority of these tumors being high grade, perioperative chemotherapy is an option, particularly with regard to the benefit in terms of overall survival recently demonstrated with this approach.
