Treatment of Ulcerative Colitis in Mildly to Moderately Active Ulcerative Proctitis

This protocol defines the first-line management approach for patients with mildly to moderately active ulcerative proctitis — a distal presentation of ulcerative colitis with specific therapeutic recommendations and clearly defined remission targets.

Clinical scenario: Mildly to moderately active ulcerative proctitis

Treatment Approach

The recommended approach for induction of remission in this setting centres on a rectal therapy from the 5-aminosalicylate acid class. The complete protocol — including the specific formulation, delivery method, and sequencing — is available via the link below.

Clinical Goals

Induction therapy is reassessed within 8 weeks. Remission is defined by all of the following:

References

  1. In patients with mildly to moderately active ulcerative proctitis, we recommend rectal 5-aminosalicylate acid (5-ASA) therapies at a dose of 1 g/daily for induction of remission
  2. Patients with mildly to moderately active UC should be reassessed to determine response to induction therapy within 8 weeks.
  3. In clinical trials, the definition of clinical remission has been a MES of 0 or 1, stool frequency of ≤1, and absence of rectal bleeding.
