Trigeminal Neuralgia When Carbamazepine or Oxcarbazepine Fails to Achieve Pain Remission
This protocol addresses patients with trigeminal neuralgia who have undergone first-line anticonvulsant therapy and have not achieved the expected treatment goal. A structured next-line approach is warranted when the defined remission target is not met.
The patient was treated with a first-line anticonvulsant — carbamazepine or oxcarbazepine — but did not reach the required target: remission with no pain for at least 4 weeks. Failure to achieve sustained pain-free remission is the trigger for escalation to this next treatment step.
When first-line therapy has not achieved adequate pain control, the next step involves the use of an adjuvant anticonvulsant — either in combination with the prior agent or as monotherapy. The specific selection, sequencing, and full regimen are set out in the structured protocol.
Should a medication be ineffective, then another adjuvant medication may be added; however, there is little evidence assessing this approach.
