Treatment of Tracheal Cancer in Adenoid Cystic Carcinoma with Low Disease Dynamics

Adenoid cystic carcinoma (ACC) is one of the most common primary tracheal malignancies, alongside squamous-cell carcinoma, together accounting for more than two-thirds of primary tracheal tumors in adults. In patients with ACC presenting with low disease dynamics, the management approach differs meaningfully from more rapidly progressive disease.

Clinical Scenario

Advanced adenoid cystic carcinoma of the trachea with low disease dynamics. In this setting, the indolent pace of the tumor informs both the selection of therapy and the expected timeline of response.

Approach (partial overview)

For patients with ACC and low disease dynamics, a cytostatic monotherapy strategy is an established option. In more advanced or later-line settings, molecular profiling-guided targeted therapy — including next-generation sequencing — may be used off-label to identify targeted agents when standard options have been exhausted.
