This protocol addresses a specific presentation of advanced tracheal cancer: adenoid cystic carcinoma (ACC) of the trachea in the setting of low disease dynamics — where the slow pace of progression is the defining feature shaping the systemic treatment approach.
Adenoid cystic carcinoma is one of the two most prevalent primary tracheal malignancies in adults, alongside squamous-cell carcinoma — together accounting for more than two-thirds of such cases. When ACC presents in the advanced setting with low disease dynamics, the indolent tempo of progression becomes the central determinant of treatment selection, distinguishing this scenario from more rapidly progressive disease.
For ACC of the trachea with low disease dynamics, the evidence-based strategy centres on a cytostatic approach — specifically, a single-agent chemotherapy regimen. The slow-growing biology of ACC and considerations around tolerability inform this direction.
