Treatment of Thymic Carcinoma at Masaoka-Koga Stage IVB (TNM Stage IVB) with Lymphogenous or Haematogenous Distant Metastasis

Clinical Scenario

This protocol applies to thymic carcinoma at Masaoka-Koga stage IVB (TNM stage IVB) — the most advanced stage of thymic epithelial tumour — defined by the presence of lymphogenous or haematogenous (distant) metastasis.

Stage IVB Disease Context

At stage IVB, disease is advanced, non-resectable, and metastatic. Systemic treatment as a single modality is indicated in patients whose tumour cannot be surgically removed or definitively irradiated.

  • Treatment algorithm for metastatic thymic tumour (Masaoka-Koga stage IVB, TNM stage IVB).
  • Chemotherapy should be offered as single modality treatment in advanced, non-resectable, non-irradiable or metastatic (stage IVB) thymic epithelial tumours.
  • Sunitinib may then represent an off-label option as second-line treatment of thymic carcinomas, independently from KIT status [III, A].
  • Everolimus may, therefore, represent an off-label option for refractory tumours [III, B].
  • It is not recommended to administer imatinib in the absence of a KIT-sensitising mutation, following the report showing the absence of activity in two phase II trials conducted in this setting [III, E].
