Clinical Scenario

This protocol addresses seminoma germ cell tumour at clinical stage IIA or IIB, characterised by retroperitoneal lymph node metastasis with a lymph node mass up to 5 cm in greatest dimension.

Previous Line — Escalation Trigger

The preceding line of treatment was salvage chemotherapy (VIP, TIP, or GIP). Escalation to this protocol is indicated when that line failed to achieve normalization of serum tumour markers or a complete or partial radiological response.

Next-Line Approach (Partial Summary)

This protocol employs a high-dose chemotherapy strategy combined with autologous stem cell transplantation. The specific agent combination, sequential cycle structure, and full clinical algorithm are available in the complete protocol.

Treatment Goals

Normalization of serum tumour markers and complete radiological response.

Historically, radiotherapy has been the primary treatment for stage II A/B seminoma, showing relapse rates between 5–11%.
Chemotherapy is a standard option for stage IIA/B seminoma.
HDCT with autologous stem cell support should be used although the prospect of cure is < 25%.
When HDCT is used as a salvage treatment, sequential treatment cycles of high-dose carboplatin and etoposide (HD-CE) should be preferred to a single high-dose regimen as the former is associated with less toxicity-related deaths.
