Treatment of Troublesome Tardive Dyskinesia Interfering With Daily Activities and Quality of Life

Clinical Scenario

This protocol applies to patients with an approved indication for antipsychotic use who have developed tardive dyskinesia that is at least moderate in severity and significantly interferes with activities of daily living and quality of life.

Treatment Approach

Clinical Goal

The protocol targets at least 50% improvement on standard tardive dyskinesia rating scales within 6 months of initiating the intervention, with evidence supporting sustained long-term benefit.

In patients with troublesome TS, suppressive agents should be considered.
In patients whose TS is troublesome and refractory to oral medications, DBS is currently recommended as possibly effective.
All available studies indicate that GPi is a preferred target for patients with TD, demonstrating a minimum of 50% improvement in standard rating scales observed as early as 6 months following treatment, with sustained benefits in the long-term.
GPi DBS should be reserved for intractable TD.
