This protocol covers the management of syndrome of inappropriate antidiuretic hormone secretion (SIADH) in patients where acute hyponatraemia is documented to have existed for fewer than 48 hours, in the absence of severe or moderately severe symptoms.
The patient presents with acute hyponatraemia confirmed within a 48-hour window. The short, documented duration of sodium imbalance is the defining feature of this scenario and shapes the clinical approach. Severe and moderately severe symptoms are not present.
Where possible, the initial step involves withdrawing fluids, medications, and other factors that may be contributing to or provoking hyponatraemia, alongside prompt diagnostic assessment and cause-specific treatment. In cases where a significant acute fall in serum sodium concentration is identified, intravenous saline correction may form part of the initial response.
