Treatment of Subarachnoid Hemorrhage with Depressed Consciousness from Large Intraparenchymal Hematoma

This protocol applies to patients with aneurysmal subarachnoid hemorrhage who are deemed salvageable but present with a depressed level of consciousness attributable to a large intraparenchymal hematoma.

Aneurysmal subarachnoid hemorrhage (aSAH) deemed salvageable, with depressed level of consciousness due to large intraparenchymal hematoma. In this setting, emergency intervention is indicated to reduce mortality.

Management involves emergency surgical intervention targeting rapid clot evacuation — the full structured regimen, including the surgical strategy and timing considerations, is available in the complete protocol.

For patients with aSAH deemed salvageable and with depressed level of consciousness due to large intraparenchymal hematoma, emergency clot evacuation should be performed to reduce mortality.

The desire for rapid clot evacuation generally favors surgery without delay and concomitant aneurysm clipping.
