Treatment of Gastric Cancer with Oligometastases Confined to the Liver

Gastric cancer presenting with oligometastases confined to the liver represents a specific clinical scenario in which the hepatic compartment is the sole site of distant spread. This pattern of disease defines a distinct patient population and informs how management is approached.

Clinical Scenario

This protocol applies to patients with gastric cancer and oligometastatic disease limited to the liver. Careful patient selection is a defining feature of eligibility for the approach described in the full protocol.

Treatment Approach (Overview)

For appropriately selected patients, management involves a combined surgical and systemic strategy. The complete regimen, selection criteria, and sequencing are detailed in the structured protocol.

Radical gastrectomy, metastatectomy and systemic therapy may be considered for selected gastric cancer patients with oligometastases in the liver.

The meta-analysis showed that radical gastrectomy with hepatectomy plus chemotherapy provided a survival benefit compared to chemotherapy alone (HR, 0.27; 95% CI, 0.12 to 0.62; P=0.002).
