Treatment of Advanced or Metastatic, Untreated HER2-Negative Gastric Cancer with PD-L1 CPS ≥5

This protocol covers the initial systemic treatment of advanced or metastatic gastric cancer in patients who are HER2-negative and whose tumour expresses PD-L1 with a combined positive score (CPS) of 5 or more.

Clinical scenario

Advanced or metastatic disease — no prior systemic therapy — HER2-negative status confirmed — PD-L1 CPS of 5 or more on tumour testing.

The full protocol specifies regimens that include trifluridine-tipiracil among the options addressed, alongside additional agents. Complete sequencing, selection criteria, and evidence ratings are available in the structured protocol below.

  • NivolumabeChT is recommended for advanced, untreated gastric, OGJ and oesophageal cancer with a PD-L1 CPS 5 [I, A; ESMO-MCBS v1.1 score: 4].
  • For patients previously treated with two lines of therapy, trifluridine–tipiracil is recommended [I, A; ESMO-MCBS v1.1 score: 3]. Alternative treatments include a taxane or irinotecan [II, B].
