Treatment of Sporotrichosis in Children Under 18 with Cutaneous or Lymphocutaneous Disease

This protocol covers the management of sporotrichosis specifically in paediatric patients — those under 18 years of age — presenting with cutaneous sporotrichosis or lymphocutaneous sporotrichosis.

Clinical scenario

Patients under 18 years of age with cutaneous sporotrichosis or lymphocutaneous sporotrichosis. The paediatric age group requires a specific therapeutic approach distinct from adult management.

Treatment approach

An oral antifungal agent is recommended as first-line therapy for these children. A separate oral alternative is also available for paediatric patients in whom the first-line option is not appropriate.

  • Itraconazole, administered at a dosage of 6–10 mg/kg up to a maximum of 400 mg orally daily, is recommended for children with cutaneous or lymphocutaneous sporotrichosis (B-III).
  • An alternative for children is SSKI therapy, which should be initiated at a dosage of 1 drop (using a standard eye-dropper) 3 times daily and increased as tolerated up to a maximum of 1 drop/kg or 40–50 drops 3 times daily, whichever is lowest (B-III).
