Spondylolysis is a stress fracture or defect of the pars interarticularis, typically presenting as activity-related low back pain. This protocol addresses nonoperative management in patients presenting with spondylolysis.
Management is built around activity modification — including discontinuation of sporting activity — alongside a structured, progressive physical therapy program and pharmacological pain control. The protocol specifies a multi-stage rehabilitation sequence and defines the conditions under which additional interventions are introduced.
The primary targets at 6 weeks are resolution of low back pain and normalized lumbar active range of motion. These benchmarks guide progression through rehabilitation and the return to sporting activity.
