Treatment of Splenic Vein Thrombosis in Splanchnic Vein Thrombosis with Portal Hypertension

Clinical Scenario

This protocol addresses patients with splanchnic vein thrombosis who have established portal hypertension or are considered to be at risk of developing it.

Key Concern

In this setting, variceal risk is a primary management priority. Early endoscopic assessment is recommended to screen for varices, followed by targeted prophylaxis for those at high risk of variceal bleeding.

Approach — Partial Overview

Management involves early endoscopic evaluation for variceal screening. Where high-risk varices are identified, specific prophylactic interventions are indicated. The complete protocol — including the full sequence, criteria, and intervention details — is available via the link below.

In patients with splanchnic vein thrombosis who have or are considered to be at risk of portal hypertension, we recommend early esophagogastroduodenoscopy for variceal screening followed by adequate medical prophylaxis and endoscopic variceal band ligation of high-risk varices.
