Treatment of Splenic Vein Thrombosis with Peritonitis, Shock, or High Lactate

This protocol addresses splenic vein thrombosis occurring in the context of acute splanchnic vein thrombosis presenting as a critical emergency — a situation that demands a distinct and urgent management sequence.

The patient presents with acute splanchnic vein thrombosis accompanied by one or more of the following: haemodynamic shock, markedly elevated lactate, signs of peritonitis, intestinal perforation, intestinal infarction, or acute major gastrointestinal bleeding. This combination creates an immediate surgical emergency.

Immediate surgical evaluation is the priority in this setting — the sequencing of any anticoagulant treatment is directly shaped by this surgical step. The complete management algorithm is available via the structured protocol.

In patients with acute splanchnic vein thrombosis presenting with shock, high lactate levels or signs of peritonitis, perforation, intestinal infarction, or acute major gastrointestinal bleeding we recommend immediate surgical evaluation and treatment before anticoagulation is introduced.
