Treatment of Solid Papillary Carcinoma of the Breast with Invasion in Pre-Menopausal, Oestrogen Receptor Positive Women

Clinical Scenario

This protocol covers the treatment of solid papillary carcinoma of the breast with evidence of invasion in a specific population: pre-menopausal women whose tumour is oestrogen receptor (ER) positive.

All patients with ER positive disease should receive endocrine therapy. Pre-menopausal status further shapes the approach, as hormonal factors remain active and must be addressed in the treatment plan.

Treatment Approach

The cornerstone of management in this population is endocrine (hormone-based) therapy, given over an extended duration. For certain pre-menopausal women, ovarian suppression or ablation may be considered as an additional measure alongside the primary endocrine treatment.

  1. All pre-menopausal women should receive at least 5 years of tamoxifen 20 mg once a day.
  2. All patients with ER positive disease should, therefore, receive endocrine therapy.
  3. Ovarian suppression / ablation should be offered using gosarelin 3.6 mg subcutaneously once monthly for 2 years in addition to tamoxifen in pre-menopausal women with ER positive early invasive breast cancer who have been offered chemotherapy but have chosen not to have it.
