This protocol addresses Sjögren syndrome in patients who have positive serum cryoglobulins with symptomatic cryoglobulinemia and evidence of active systemic disease — a distinct subset requiring targeted management.
Sjögren syndrome presenting alongside symptomatic cryoglobulinemia and positive serum cryoglobulins in the context of active systemic disease. The best indication for intervention is for symptoms linked to cryoglobulinemia-associated vasculitis. Treatment of systemic disease is tailored to organ-specific severity using established disease activity definitions.
The approach to active systemic disease in this setting involves glucocorticoid therapy, with the specific route and intensity guided by disease severity. Glucocorticoids are used at the minimum dose and duration necessary to control active disease.
The complete regimen — including severity-stratified dosing guidance and sequencing — is available in the full protocol below.
The primary therapeutic target is a reduction of ≥3 points in the global ESSDAI (EULAR Sjögren's Syndrome Disease Activity Index) score, reflecting meaningful control of systemic disease activity.
