Sigmoid Volvulus in Pregnancy: Clinical Management Approach

Colonic sigmoid volvulus is a recognised but uncommon complication that can occur during pregnancy, presenting unique diagnostic and therapeutic challenges for the clinical team.

Clinical scenario: Sigmoid volvulus arising in a pregnant patient. This is a rare presentation. Management must take into account both the patient's condition and the stage of pregnancy, requiring close coordination across specialties.
Treatment approach (partial overview): For uncomplicated cases, an endoscopic approach may be considered as part of the initial management — though effectiveness can vary depending on gestational stage. The complete stepwise regimen, including the full range of options and decision points, is available via the structured protocol.

For uncomplicated sigmoid volvulus, endoscopic detorsion is recommended but may be ineffective especially in the third trimester because of the volume of the uterus.
