This protocol addresses the clinical scenario in which sialolithiasis of the sublingual gland presents alongside sialolithiasis of the parotid gland where the stone is located in the papilla or the distal excretory duct — a location that has direct implications for the choice of retrieval approach.
Clinical scenario: Concurrent sialolithiasis of the parotid gland with the stone situated in the papilla or distal excretory duct. The anatomical position of the parotid stone at this level shapes the therapeutic strategy for both glands.
Management in this setting involves a transoral approach directed at the retropapillary region — potentially supplemented by endoscopic intervention — or a combined transcutaneous–endoscopically guided retrieval technique. The full selection criteria, procedural sequence, and decision algorithm are detailed in the complete protocol.
Stones in the Papilla and Distal Excretory Duct
If the anatomy is unsuitable or ISWL is not available, a combined retropapillary transoral approach, if necessary combined with intSE with or without stent implantation, is another reasonable solution.
Combined transcutaneous–endoscopically guided stone retrieval is another modality in this location.
