Treatment of Shoulder Osteoarthritis Without NSAID Contraindications or Intolerances

This protocol addresses shoulder osteoarthritis in patients who have no contraindications or intolerances to NSAIDs — a subgroup where the broader pharmacologic landscape remains open. Even so, the structured regimen for this scenario extends beyond simple NSAID use.

Clinical scenario: Oral and topical NSAIDs are among the pharmacologic agents most recommended for osteoarthritis. This protocol specifically applies when neither formulation is ruled out by contraindications or intolerances — yet the management of the shoulder joint involves additional considerations that shape the treatment approach.
Treatment approach (partial): The regimen for this scenario includes an intra-articular injection directed at the shoulder joint — the full protocol, including specifics and sequencing, is accessible via the link below.

Oral and topical NSAIDs are the pharmacologic agents most recommended for OA, though oral formulations should be avoided in those with contraindications and intolerances, and topical formulations are not appropriate for a deep joint such as the hip.

IACS may be considered, whereas other intra-articular therapies were generally not recommended, or in the case of IAHA, only to be considered if other therapies have failed.
