Treatment of Sepsis or Septic Shock with High Risk for Multidrug-Resistant (MDR) Organisms

Adults presenting with sepsis or septic shock in the context of high risk for multidrug-resistant (MDR) organisms represent a distinct clinical population. Standard empiric antimicrobial choices applied to routine sepsis may be insufficient in this setting.

This protocol is for adults with sepsis or septic shock who carry a high risk for multidrug-resistant (MDR) organisms. The presence of septic shock in this group further underscores the need for a deliberate, evidence-guided approach to empiric gram-negative coverage.

Current evidence supports the use of a dual-agent empiric strategy directed at gram-negative pathogens in this population — specific agent selection and the full clinical algorithm are available in the structured protocol.

For adults with sepsis or septic shock and high risk for multidrug resistant (MDR) organisms, we suggest using two antimicrobials with gram-negative coverage for empiric treatment over one gram-negative agent.
