Treatment of Secondary Syphilis with Neurological Involvement

Secondary syphilis can affect the nervous system, creating a clinical scenario that requires a more intensive management approach than uncomplicated disease. This protocol addresses cases where neurological involvement has been identified.

Patients can develop neurological complications during secondary syphilis. When neurological involvement is present, the treatment strategy and follow-up differ substantially from standard secondary syphilis management, with close monitoring of cerebrospinal fluid parameters as a central component.

First-line management is based on parenteral penicillin therapy. Alternative antibiotic regimens exist for patients unable to receive first-line treatment. Steroid co-administration alongside antibiotic therapy is incorporated into the complete protocol for this indication. Full dosing, sequencing, duration, and route details are contained within the structured regimen.

Treatment Goals Decrease in cerebrospinal fluid white blood cell count and protein level, assessed by CSF examination 6 weeks to 6 months after treatment.
