In some patients with secondary hypothyroidism, repeated cycles of levothyroxine dose adjustment do not bring the TSH level into the target range. When that goal is not met despite multiple adjustment attempts, a defined next-line protocol applies.
The prior approach consisted of levothyroxine dose adjustment, repeated every six to eight weeks. The treatment goal — a TSH level in the normal range (0.4 to 4.5 mIU/L) — was not achieved after two or more adjustment cycles. This failure to reach target is the trigger for escalation to this protocol.

If the TSH level or symptoms are not improving after two to three cycles of adjustments, referral to endocrinology may be considered after reassessment of the differential diagnosis, patient adherence, and drug-drug or drug-food interactions.
