Secondary erythema nodosum presenting with tender erythematous nodules on the lower extremities that have not resolved with standard first-line measures. This protocol defines the structured next step when initial management proves insufficient.
Initial management with compression bandages and limb elevation for edema and pain relief, combined with indomethacin or naproxen for pain control, did not achieve the expected outcome: resolution of tender erythematous nodules on the lower extremities within 1–6 weeks. This failure to reach the treatment target is the trigger for escalation.
