Scalp Seborrheic Dermatitis in Adults: Next-Line Management After Systemic Antifungal Therapy Has Failed

Adult patient with seborrheic dermatitis of the scalp whose prior systemic antifungal treatment course did not achieve the expected clinical response.

The preceding line used systemic antifungal therapy — terbinafine or itraconazole. The targets that were not reached: prompt reduction of SD symptoms and skin inflammation, and complete clearing or marked improvement of scalp SD. Failure to achieve these goals is the criterion that escalates care to this next-line protocol.

This protocol employs a light-based (phototherapy) intervention for resistant or severe scalp seborrheic dermatitis in adults. The specific modality, session schedule, and full treatment parameters are detailed in the complete structured protocol.

However, limited clinical evidence supports the advantages of UVB phototherapy (Level C) in diffuse and resistant SD forms.

There is only one prospective, open-label clinical study showing narrow-band UVB phototherapy, three times weekly up to a maximum of 8 weeks, to be effective in improving severe SD, with complete clearance in 6 patients and marked improvement in 12.
