Sarcoidosis may co-occur with small fiber neuropathy, a combination that introduces symptoms not well addressed by standard sarcoidosis regimens and that requires a distinct management approach.
A number of hitherto unexplained symptoms such as fatigue, pain, and cognitive dysfunction may — at least partly — be attributable to small fiber neuropathy. Standard anti-inflammatory therapies are usually ineffective for this condition.
In refractory cases, targeted immunomodulatory strategies — including anti-TNF agents and/or immunoglobulin-based therapy — have been reported in selected patients.
