Treatment of Sarcoidosis with Posterior Uveitis or Panuveitis

Clinical Scenario

This protocol addresses sarcoidosis presenting with posterior uveitis or panuveitis — ocular involvement that typically requires systemic rather than local therapy alone.

Why Systemic Therapy

For posterior uveitis or panuveitis in the setting of sarcoidosis, systemic therapy is usually indicated. Posterior segment involvement carries a greater risk to vision and generally cannot be managed by topical measures alone.

Treatment Approach

When adequate disease control requires more than low-dose corticosteroids, corticosteroid-sparing agents are added. The approach involves a cytotoxic agent alongside corticosteroids — the specific agent and regimen are selected based on the individual clinical picture.

Full regimen details, agent selection, and sequencing are in the complete protocol.

References

  1. For posterior uveitis or panuveitis, systemic therapy is usually used.
  2. If greater than 10 mg prednisone is needed to control the disease, then corticosteroid-sparing drugs should be used.
  3. Cytotoxic drugs such as methotrexate, azathioprine, and mycophenolate mofetil have been used with success.
