Pulmonary Sarcoidosis with Dyspnea and Stage II+ Chest X-Ray: When Steroid-Sparing Therapy Has Not Achieved Clinical Effectiveness

Clinical Scenario

This protocol addresses patients with pulmonary sarcoidosis presenting with dyspnea, chest X-ray findings at stage II or higher, and pulmonary function test results below normal. Ongoing breathlessness alongside measurable radiographic and functional impairment signals that the current treatment approach is not sufficient.

Prior Treatment — Clinical Effectiveness Not Achieved

The preceding treatment line added a steroid-sparing immunosuppressant — methotrexate, azathioprine, mycophenolate, or leflunomide — targeting clinical effectiveness within 6 months. When that outcome has not been reached, escalation to the next treatment step is warranted.

Next-Line Treatment Approach
