Cutaneous sarcoidosis presents with specific granulomatous skin lesions that vary widely in extent. When the disease is limited to a small number of localized lesions — particularly those that are cosmetically important — the clinical approach differs from more widespread or systemic involvement.
This protocol applies to patients with cutaneous sarcoidosis presenting with specific granulomatous skin lesions that are few and localized, with cosmetic significance. Patients with very few localized lesions may respond to topical or locally delivered interventions before broader systemic strategies are considered.
When localized lesions do not respond adequately and the disease becomes more severe, a biologic anti-TNF strategy may have a role in management.
If the patient has very few localized lesions, they may respond to application of a corticosteroid cream or intralesional injections.
For very severe cases, anti-TNF therapy, such as infliximab may have a role.
