Retinal Hemangioblastoma Arising at the Optic Disc (Juxtapapillary / Epipapillary)

Retinal hemangioblastoma (RH) arising in direct association with the optic disc — classified as juxtapapillary or epipapillary — represents a distinct anatomical subtype from tumours located elsewhere in the retina, and presents its own specific management considerations.

In this scenario the lesion is sited adjacent to or overlying the optic disc. This location sets it apart from extrapapillary RH and substantially affects which interventions can safely be considered.

Clinical Approach

The proximity to the optic disc and the limitations of existing ablative therapies in this region mean that a conservative strategy plays a central role — though the full structured protocol addresses the broader set of clinical considerations that go beyond watchful waiting alone.

RH can arise in association with the optic disc (designated as juxtapapillary or epipapillary RH), or within retina elsewhere (designated as extrapapillary RH).

Given limitations to existing therapies, observation can be a viable strategy.
